The review presents the history of the development of ideas about the role of microangiopathy in pathogenesis of the chronic diabetes complications. The tendency to shift the focus from the study of morphological changes to functional disorders of neurological regulation is demonstrated. Modern views on the pathology of microcirculation in diabetic patients are presented. The main functional tests for the study of neurocirculatory regulation with their application in clinical practice are described.
