Клинико-морфологическая характеристика фенотипов атопического дерматита

Обоснование: Атопический дерматит (АтД) представляет собой хроническое воспалительное заболевание кожи с выраженной клинической и патогенетической гетерогенностью. Особенно вариабельными являются взрослые формы АтД, для которых отсутствуют четкие диагностические критерии, что усложняет верификацию диагноза и планирование терапии. Целью данного обзора является систематизация современных представлений о клинико-морфологических фенотипах взрослых форм заболевания, включая нуммулярный, фолликулярный, пруриго-подобный/пруригинозный АтД, а также АтД кистей, экзему сосков и дерматит головы и шеи.
Методы: Проведен анализ отечественных и зарубежных источников, индексированных в базах данных PubMed, Scopus, Web of Science и eLibrary за период 2021–2026 гг. Критериями отбора являлись оригинальные исследования и обзорные статьи, посвященные клиническим фенотипам АтД, патоморфологии кожи, генетическим и иммунологическим механизмам развития заболевания.
Результаты: В обзоре систематизированы особенности различных форм АтД. При нуммулярной форме выявляются очаги спонгиоза с лихеноидной инфильтрацией и смешанным Th2/Th17/Th22-профилем. Фолликулярный АтД характеризуется перифолликулярным акантозом и гиперкератозом без спонгиоза. Пруриго-подобная (пруригинозная) форма отличается эпидермальной гиперплазией, фиброзом дермы и высокой экспрессией IL-31. Помфоликс проявляется спонгиозом вокруг эккринных протоков с участием IL-1 и IL-31. Экзема сосков проявляется акантозом, паракератозом и лихенификацией в области ареол. Дерматит головы и шеи ассоциирован с Th2-ответом и колонизацией Malassezia.
Заключение: Представленные в обзоре данные свидетельствуют о необходимости фенотипирования АтД с учетом морфологических и патогенетических особенностей. Это может служить основой для дифференцированного подхода к терапии, позволяя оптимизировать лечебную тактику в зависимости от доминирующего механизма развития заболевания.
