Целесообразность эмпирической антибактериальной терапии при инфекционном уретрите у мужчин: экспертное мнение дерматовенеролога и медицинского юриста

Инфекционный уретрит у мужчин (воспаление мочеиспускательного канала) вызывается различными возбудителями, передаваемыми половым путем (ИППП) – чаще всего бактериями, такими как Chlamydia trachomatis, Neisseria gonorrhoeae или Mycoplasma genitalium. В клинической практике, нередко еще до получения результатов выявления возбудителя методами лабораторной диагностики, назначается эмпирическая антибиотикотерапия при подозрении на инфекционный уретрит. В условиях глобально увеличивающейся антибиотикорезистентности ИППП важно учитывать эпидемиологию возбудителей, их чувствительность к лекарственным препаратам и возможные сопутствующие инфекции. Лабораторная диагностика ИППП при подозрении на инфекционный уретрит включает: при наличии уретрального отделяемого – взятие мазка из уретры для микроскопического исследования, бактериологического посева и определения чувствительности N. gonorrhoeae к антибиотикам; вне зависимости от наличия выделений – выполнение метода амплификации нуклеиновых кислот на выявление ИППП (например, N. gonorrhoeae, C. trachomatis и M. genitalium)
в соскобе из уретры. Вопрос о начале эмпирической антибиотикотерапии уретрита должен решаться индивидуально,
с учетом определенных критериев у пациента: тяжесть симптомов, риск осложнений, эпидемиологические данные и т.д. При подозрении на гонококковый уретрит рекомендуется назначение цефтриаксона. При подозрении на негонококковый уретрит рекомендуемым препаратом для эмпирического лечения является доксициклин и джозамицин. Аналитический обзор содержит подробные рекомендации по эмпирической антибиотикотерапии при инфекционном уретрите. Авторы анализируют основные критерии, указывающие на целесообразность или нежелательность проведения эмпирической антибактериальной терапии.
