Dynamics of quality of life in patients with different forms of epilepsy against a background of therapy with topiramate
Assessment of therapeutic efficacy and tolerability of topiramate (Topamax), its effect on emotional state, sleep and quality of life is presented. We examined 45 patients (18 men and 27 women) with average age 30.7 ± 11.7 years. Topiramate administration started with a dose of 25-50 mg daily, titration step was 3-5 days. The effective dose was 50-500 mg daily (1-5 mg/kg). The study included history taking, general clinical and neurological examinations, analysis of EEG, X-ray and/or magnetic resonance imaging of the brain, comparative evaluation by next scales: subjective sleep characteristics score, sleep apnea syndrome screening, Epwort sleepiness scale, Hospital Anxiety and Depression Scale, British National Hospital Scale severity of epileptic seizures, QOLIE-89 scale. Remission was achieved in 22.2% of cases. Side effects occurred in 17.7% of patients. The greatest effect was marked in cases of secondary generalized seizures. The link between quality of life and side effects of therapy, psychoemotional and sleep disorders is noted.
