Клинико-иммунологические портреты атопического дерматита. Обзор литературы

Атопический дерматит (АтД) представляет собой хроническое воспалительное заболевание кожи. За последние десятилетие распространенность АтД значительно выросла. И, согласно мировым данным, в настоящее время АтД наблюдается примерно у 20% детей и 2–5% взрослых. В зависимости от возрастных особенностей имеются различия в клинической картине, патогенезе и морфологии АтД. Эти особенности учитываются при выделении фенотипов АтД, которые представляют собой набор индивидуальных характеристик, являющихся результатом взаимодействия генетических, иммунологических и экологических факторов. Стоит отметить, что единой системы определения фенотипов АтД в настоящее время не существует. Известно, что у пациентов с АтД наблюдаются различные кожные проявления, некоторые из которых называются стигматами и представляют собой характерные признаки АтД. Но в литературе были описаны и несколько других фенотипов АтД, характеризующихся атипичными проявлениями, например нуммулярная экзема, узловатое пруриго, лихенифицированный дерматит и фолликулярный/папулезный дерматит и др. Относительно недавно были выявлены и новые состояния, сопровождающие АтД: амилоидозный лишай, витилиго, хондродерматит ушной раковины и ангиогистиоцитоидные папулы. В данном обзоре представлены различные варианты фенотипов АтД, включающие в себя клинико-иммунологические особенности.
